This page provides access to forms that may be needed for filing claims or updating enrollment. Employees and their dependents can access their claim information by Logging in to MCMC Interactive. If you need an ID card, Prescription card, or have a request please go to the online Request Page . Claim Forms - As a general rule, Medical Claims Management Corporation needs a claim form once a year from the participant in the Plan. In the case of an accident a claim form is needed to determine if any accident benefits apply. If a Spouse files a claim we may request a claim form to determine if other insurance is in force and needs to be coordinated with. Applications - An application needs to be completed for making any change to your coverage. This includes adding and deleting dependents. All applications need to be routed through your Human Resources Department. Student Status - For children over the age of 18 a form needs to be completed to determine if coverage may be allowed under your Plan's eligibility provisions. Dependent Verification - This form is used to determine if a dependent may be covered due to legal custody, adoption, guardianship, or divorce/separation legal agreement. |
